Apply for MHIP Coverage

It's easy to apply for MHIP coverage. Follow the steps below to make sure you're application isn't delayed.

1. Check Eligibility Requirements

   MHIP’s state pool is accepting applications for enrollment. Make sure you meet the guidelines.

2. Select Your Plan

   The state pool has five plans to choose from. Review the plans to select the one that best fits your needs.

3. Review Premium Payments

   MHIP's plans require a monthly premium payment. The premium amount varies depending on your age and gender. Find your premium for the plan you've selected.

4. Complete Your Application

   
   
   • Download the MHIP Application.
   • Complete and sign the application.
   • Use the checklists at the end of the application to make sure that you have completed and included everything necessary to process your application. Incomplete or unsigned applications will delay your enrollment, and may result in your application being denied.

5. Send Your Application and Other Materials

   Mail your application, first month's premium, and all required documents to the address for your area, which is shown at the end of the application.
